Fernandez, who was based out of the Mission division, surrendered to authorities on Wednesday after a warrant was issued for his arrest.
He was charged with two felony counts of workers' compensation insurance fraud and insurance fraud after he
allegedly submitted inconsistent doctor's notes while off-duty on a workers' comp claim for a ten-month period between May 2010 and last March.
Investigators said the sergeant doctored the doctor's note, which allowed him to take off more time than was actually needed.
He was released on $30,000 bail.
Fernandez, who was assigned to the Mission District, could face up to seven years in state prison if convicted.
